How We Work

For individuals and families who have never received care before, the process of getting it set up can be daunting. Here at Agrade Community Care Services want to make it is as easy and stress free as possible. You may also have been referred to us by the social services department from which you initially sought help.

  1. 1 - Call our office to speak to one of our team

    Call us on 01246 252373 or email us at Our office is manned by experienced staff and they will be able to guide you through the process. Whether you just want some more information or you would like to arrange a home assessment.

  2. 2 - Arrange a free, no obligation, home assessment

    The assessment will be carried out by specially trained staff. We want to build up a full picture and we will do this as quickly and tactfully as possible Our aim is always to make sure that we understand what you need and what your preferences are , so that we can respond in ways which really suit you.

  3. 3 - Personalised care package is set up

    Once we have carried out all your assessment and you have decided to proceed with care at home, your personalised care package will be set up. This will be centred around your support needs and preferences.

  4. 4 - Care begins

    Your care will begin., one of our Care Coordinators will introduce you the care staff who will be supporting you at home. We will be in touch regularly to check how everything is going and to offer reviews where necessary.
